Breast Cancer Screening Day Information For Oak Street Health Patients (CLOSED EVENT)

Join our supportive community and gain access to resources for breast health. Our team will help guide you through the health care system and ease any fears along the way. We provide culturally sensitive breast cancer patient navigation and educate women on the importance of timely follow-up for the early detection of breast cancer, answer your questions and help you understand your breast cancer screenings, help to facilitate communication between yourself and your physicians, direct you to local resources and support, help you set up and keep track of all your health care services, help you to get answers to any of your questions in an efficient and timely manner and provide ongoing support through Survivorship Programs and other support groups.

Mammograms are an important screening exam for the prevention and early detection of breast cancer. By offering our navigations support and coordinating screening events, we are providing access to patients who are past due to their mammogram appointments, partly due to COVID-19 restrictions and wait times.
